The current usage of memblock_reserve() in init_pvh_bootparams() is done before
the .bss is zeroed, and that used to be fine when
memblock_reserved_init_regions implicitly ended up in the .meminit.data
section. However after commit 73db3abdca58c memblock_reserved_init_regions
ends up in the .bss section, thus breaking it's usage before the .bss is
cleared.
Move and rename the call to xen_reserve_extra_memory() so it's done in the
x86_init.oem.arch_setup hook, which gets executed after the .bss has been
zeroed, but before calling e820__memory_setup().

When running as PVH or HVM Linux will use holes in the memory map as scratch
space to map grants, foreign domain pages and possibly miscellaneous other
stuff. However the usage of such memory map holes for Xen purposes can be
problematic. The request of holesby Xen happen quite early in the kernel boot
process (grant table setup already uses scratch map space), and it's possible
that by then not all devices have reclaimed their MMIO space. It's not
unlikely for chunks of Xen scratch map space to end up using PCI bridge MMIO
window memory, which (as expected) causes quite a lot of issues in the system.
At least for PVH dom0 we have the possibility of using regions marked as
UNUSABLE in the e820 memory map. Either if the region is UNUSABLE in the
native memory map, or it has been converted into UNUSABLE in order to hide RAM
regions from dom0, the second stage translation page-tables can populate those
areas without issues.
PV already has this kind of logic, where the balloon driver is inflated at
boot. Re-use the current logic in order to also inflate it when running as
PVH. onvert UNUSABLE regions up to the ratio specified in EXTRA_MEM_RATIO to
RAM, while reserving them using xen_add_extra_mem() (which is also moved so
it's no longer tied to CONFIG_PV).

The Processor capability bits notify ACPI of the OS capabilities, and
so ACPI can adjust the return of other Processor methods taking the OS
capabilities into account.
When Linux is running as a Xen dom0, the hypervisor is the entity
in charge of processor power management, and hence Xen needs to make
sure the capabilities reported by _OSC/_PDC match the capabilities of
the driver in Xen.
Introduce a small helper to sanitize the buffer when running as Xen
dom0.
When Xen supports HWP, this serves as the equivalent of commit
a21211672c9a ("ACPI / processor: Request native thermal interrupt
handling via _OSC") to avoid SMM crashes. Xen will set bit
ACPI_PROC_CAP_COLLAB_PROC_PERF (bit 12) in the capability bits and the
_OSC/_PDC call will apply it.

When running as a paravirtualized guest under Xen, Linux is using
"lazy mode" for issuing hypercalls which don't need to take immediate
effect in order to improve performance (examples are e.g. multiple
PTE changes).
There are two different lazy modes defined: MMU and CPU lazy mode.
Today it is not possible to nest multiple lazy mode sections, even if
they are of the same kind. A recent change in memory management added
nesting of MMU lazy mode sections, resulting in a regression when
running as Xen PV guest.
Technically there is no reason why nesting of multiple sections of the
same kind of lazy mode shouldn't be allowed. So add support for that
for fixing the regression.

Only Xen is using the paravirt lazy mode code, so it can be moved to
Xen specific sources.
This allows to make some of the functions static or to merge them into
their only call sites.
While at it do a rename from "paravirt" to "xen" for all moved
specifiers.
No functional change.

Remove the dangerous late initialization of xen-swiotlb in

Making virt_to_pfn() a static inline taking a strongly typed
(const void *) makes the contract of a passing a pointer of that
type to the function explicit and exposes any misuse of the
macro virt_to_pfn() acting polymorphic and accepting many types
such as (void *), (unitptr_t) or (unsigned long) as arguments
without warnings.
Also fix all offending call sites to pass a (void *) rather
than an unsigned long. Since virt_to_mfn() is wrapping
virt_to_pfn() this function has become polymorphic as well
so the usage need to be fixed up.

The scaling information in subleaf 1 should match the values set by KVM in
the 'vcpu_info' sub-structure 'time_info' (a.k.a. pvclock_vcpu_time_info)
which is shared with the guest, but is not directly available to the VMM.
The offset values are not set since a TSC offset is already applied.
The TSC frequency should also be set in sub-leaf 2.

Implement support for the HVMOP_set_evtchn_upcall_vector hypercall in
order to set the per-vCPU event channel vector callback on Linux and
use it in preference of HVM_PARAM_CALLBACK_IRQ.
If the per-VCPU vector setup is successful on BSP, use this method
for the APs. If not, fallback to the global vector-type callback.
Also register callback_irq at per-vCPU event channel setup to trick
toolstack to think the domain is enlightened.

xen_remap() is used to establish mappings for frames not under direct
control of the kernel: for Xenstore and console ring pages, and for
grant pages of non-PV guests.
Today xen_remap() is defined to use ioremap() on x86 (doing uncached
mappings), and ioremap_cache() on Arm (doing cached mappings).
Uncached mappings for those use cases are bad for performance, so they
should be avoided if possible. As all use cases of xen_remap() don't
require uncached mappings (the mapped area is always physical RAM),
a mapping using the standard WB cache mode is fine.
As sparse is flagging some of the xen_remap() use cases to be not
appropriate for iomem(), as the result is not annotated with the
__iomem modifier, eliminate xen_remap() completely and replace all
use cases with memremap() specifying the MEMREMAP_WB caching mode.
xen_unmap() can be replaced with memunmap().

swiotlb-xen uses very different ways to allocate coherent memory on x86
vs arm. On the former it allocates memory from the page allocator, while
on the later it reuses the dma-direct allocator the handles the
complexities of non-coherent DMA on arm platforms.
Unfortunately the complexities of trying to deal with the two cases in
the swiotlb-xen.c code lead to a bug in the handling of
DMA_ATTR_NO_KERNEL_MAPPING on arm. With the DMA_ATTR_NO_KERNEL_MAPPING
flag the coherent memory allocator does not actually allocate coherent
memory, but just a DMA handle for some memory that is DMA addressable
by the device, but which does not have to have a kernel mapping. Thus
dereferencing the return value will lead to kernel crashed and memory
corruption.
Fix this by using the dma-direct allocator directly for arm, which works
perfectly fine because on arm swiotlb-xen is only used when the domain is
1:1 mapped, and then simplifying the remaining code to only cater for the
x86 case with DMA coherent device.

Reuse the generic swiotlb initialization for xen-swiotlb. For ARM/ARM64
this works trivially, while for x86 xen_swiotlb_fixup needs to be passed
as the remap argument to swiotlb_init_remap/swiotlb_init_late.
Note that the lower bound of the swiotlb size is changed to the smaller
IO_TLB_MIN_SLABS based value with this patch, but that is fine as the
2MB value used in Xen before was just an optimization and is not the
hard lower bound.

The IOMMU table tries to separate the different IOMMUs into different
backends, but actually requires various cross calls.
Rewrite the code to do the generic swiotlb/swiotlb-xen setup directly
in pci-dma.c and then just call into the IOMMU drivers.

Xen allows the usage of some previously reserved bits in the IO-APIC
RTE and the MSI address fields in order to store high bits for the
target APIC ID. Such feature is already implemented by QEMU/KVM and
HyperV, so in order to enable it just add the handler that checks for
it's presence.

There's no point in disabling x2APIC mode when running as a Xen HVM
guest, just enable it when available.
Remove some unneeded wrapping around the detection functions, and
simply provide a xen_x2apic_available helper that's a wrapper around
x2apic_supported.

Employ the fancy new EX_TYPE_IMM_REG to store -EFAULT in the return
register and use this to remove some Xen .fixup usage.
All callers of these functions only test for 0 return, so the actual
return value change from -1 to -EFAULT is immaterial.

Xen-pciback driver was designed to be built for x86 only. But it
can also be used by other architectures, e.g. Arm.
Currently PCI backend implements multiple functionalities at a time,
such as:
1. It is used as a database for assignable PCI devices, e.g. xl
pci-assignable-{add|remove|list} manipulates that list. So, whenever
the toolstack needs to know which PCI devices can be passed through
it reads that from the relevant sysfs entries of the pciback.
2. It is used to hold the unbound PCI devices list, e.g. when passing
through a PCI device it needs to be unbound from the relevant device
driver and bound to pciback (strictly speaking it is not required
that the device is bound to pciback, but pciback is again used as a
database of the passed through PCI devices, so we can re-bind the
devices back to their original drivers when guest domain shuts down)
3. Device reset for the devices being passed through
4. Para-virtualised use-cases support
The para-virtualised part of the driver is not always needed as some
architectures, e.g. Arm or x86 PVH Dom0, are not using backend-frontend
model for PCI device passthrough.
For such use-cases make the very first step in splitting the
xen-pciback driver into two parts: Xen PCI stub and PCI PV backend
drivers.
For that add new configuration options CONFIG_XEN_PCI_STUB and
CONFIG_XEN_PCIDEV_STUB, so the driver can be limited in its
functionality, e.g. no support for para-virtualised scenario.
x86 platform will continue using CONFIG_XEN_PCIDEV_BACKEND for the
fully featured backend driver.

Decouple XEN_DOM0 from XEN_PV, converting some existing uses of XEN_DOM0
to a new XEN_PV_DOM0. (I'm not convinced all are really / should really
be PV-specific, but for starters I've tried to be conservative.)
For PVH Dom0 the hypervisor populates MADT with only x2APIC entries, so
without x2APIC support enabled in the kernel things aren't going to work
very well. (As opposed, DomU-s would only ever see LAPIC entries in MADT
as of now.) Note that this then requires PVH Dom0 to be 64-bit, as
X86_X2APIC depends on X86_64.
In the course of this xen_running_on_version_or_later() needs to be
available more broadly. Move it from a PV-specific to a generic file,
considering that what it does isn't really PV-specific at all anyway.
Note that xen/interface/version.h cannot be included on its own; in
enlighten.c, which uses SCHEDOP_* anyway, include xen/interface/sched.h
first to resolve the apparently sole missing type (xen_ulong_t).

xen_swiotlb and pci_xen_swiotlb_init() are only used within the file
defining them, so make them static and remove the stubs. Otoh
pci_xen_swiotlb_detect() has a use (as function pointer) from the main
pci-swiotlb.c file - convert its stub to a #define to NULL.

This partially reverts commit 882213990d32 ("xen: fix p2m size in dom0
for disabled memory hotplug case")
There's no need to special case XEN_UNPOPULATED_ALLOC anymore in order
to correctly size the p2m. The generic memory hotplug option has
already been tied together with the Xen hotplug limit, so enabling
memory hotplug should already trigger a properly sized p2m on Xen PV.
Note that XEN_UNPOPULATED_ALLOC depends on ZONE_DEVICE which pulls in
MEMORY_HOTPLUG.
Leave the check added to __set_phys_to_machine and the adjusted
comment about EXTRA_MEM_RATIO.

Since commit 9e2369c06c8a18 ("xen: add helpers to allocate unpopulated
memory") foreign mappings are using guest physical addresses allocated
via ZONE_DEVICE functionality.
This will result in problems for the case of no balloon memory hotplug
being configured, as the p2m list will only cover the initial memory
size of the domain. Any ZONE_DEVICE allocated address will be outside
the p2m range and thus a mapping can't be established with that memory
address.
Fix that by extending the p2m size for that case. At the same time add
a check for a to be created mapping to be within the p2m limits in
order to detect errors early.
While changing a comment, remove some 32-bit leftovers.

Xen added this in 2015 (Xen 4.6). On x86_64 and Arm it fills what was
previously a 32-bit hole in the generic shared_info structure; on
i386 it had to go at the end of struct arch_shared_info.

The include/linux/pgtable.h is going to be the home of generic page table
manipulation functions.
Start with moving asm-generic/pgtable.h to include/linux/pgtable.h and
make the latter include asm/pgtable.h.

Patch series "mm: consolidate definitions of page table accessors", v2.
The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.
Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.
For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.
These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.
This patch (of 12):
The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.
The include statements in such cases are remove with a simple loop:
