mdadm: add xmalloc.h
Move memory declaration helpers outside mdadm.h. They seems to be
useful so keep them but include separatelly. Rework them to not reffer
to Name[] declared internally in mdadm/mdmon.
This is first step to start decomplexing mdadm.h.
